I hereby authorize The Cradle to publish my photograph, my name and likeness, for use in any and all mediums for the promotion of The Cradle. I hereby release and hold harmless The Cradle from any reasonable expectation of privacy or confidentiality associated with the materials specified above. I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the use of my identity or likeness in all media, including but not limited to photographs, videos, websites, emails, and promotional materials and agree that any uses described herein may be made without additional compensation or consideration of me.
