Home Study Process and Forms

We are so delighted you chose to The Cradle to help you build your family. Below is a quick overview of the home study process.

The Cradle facilitates domestic and intercountry adoptions — whether it is a first adoption or a subsequent one. Click on the buttons below to learn more about each type and access the paperwork you need.

Important Tips:

  • For fillable PDFs, you will need to download and save the form prior to entering information. Should you enter information prior to downloading, the form will not save.
  • Please only send saved or scanned documents. Do NOT send pictures of any form, ID, or documentation, as the information is not able to be seen clearly. Any pictures sent will be returned and a scanned or completed document will be requested.
  • Download or print the Home Study Adoption Process Flow Sheet so you can review and have at your fingertips.

Domestic Adoption

First Adoption

To schedule an initial consultation with a counselor, please submit the following:

 

TWO DAYS after your initial consultation, you must return the following documents to your counselor:

Before signing up for your home study classes, send the following to your counselor:

Historical and Background

 
Financial
  • Photocopy of federal tax forms for the last three years (Federal Form 1040 only)
  • Current pay stub(s) or letter from employer(s) with annualized salary (per applicant)
  • Balance Sheet
 
Verification Documents
  • State issued ID (per applicant)
  • Birth certificate (per applicant)
  • Marriage certificate (per applicant)
  • Divorce certificate(s) (if applicable)
  • Proof of legal residence (e.g., resident card, visa, certificate of naturalization, if applicable)
 

Send the following to your counselor prior to scheduling your home visit.

Clearances and Background (results needed prior to home visit)

 

Health Information


Training

  • Infant/Child CPR Certification card(s) (per applicant)
  • Adoption 101 homework (per applicant)
  • Identity homework (per applicant)
  • Complete required ALP classes (see Education Checklist)

Agency Assisted Adoption

  • Agency Assisted Flow Chart
  • Interstate Compact on Adoptable Children (ICPC)
  • Suggested Placing Agencies

Subsequent Adoption – Domestic

Send the following documents to your counselor to complete the formal Home Study process.

  • Photocopies of state issued ID, birth, marriage, and divorce certificates
  • Photocopy of federal tax forms for last 3 years (Federal Form 1040 only)
  • Training Certificates (ALP and live classes)
  • Infant/Child CPR Certification Card (one per applicant)
  • Copy of well water compliance with Health Department (if applicable)
  • Copy of current pet inoculation(s) (if applicable)
  • Current pay stub(s) or letter from employer(s) with current annualized salary. If self-employed, letter from CPA may be required.
  • Documentation from medical insurance carrier stating how coverage is initiated for an adopted child
  • Letter(s) from health professional(s), if requested.
  • Documentation of police reports/court dispositions, if requested.

These forms will be emailed to you prior to your initial meeting. Please review and bring to your counselor at the initial meeting.

After completing Parenting Through Adoption: 101 and Parenting Through Adoption: Identity – Send the following completed and signed forms to your counselor.
